Patient history and context

Understanding the patient starts with asking the right questions.

Functional medicine seeks to understand the patient beyond the immediate symptom or reason for the visit: medical history, nutrition, digestion, sleep, lifestyle, environment, exposures, symptoms and family history can all provide relevant context for clinical assessment. Collecting that depth requires structure.

You decide what you need to know. Oolara helps you organize it.

Depth

A clinical history that can look beyond the reason for the visit.

These are the families included in the functional medicine area. Each one groups several sections, and you choose which ones your practice uses.

  • Identity and medical historyGeneral information, reason for the visit, allergies, family history and medical history
  • LifestyleNutrition, stress, stress tolerance, sleep and hydration
  • Digestive systemGastric and biliary function, bowel habits, SIBO/dysbiosis and intestinal barrier
  • Energy and metabolismGlycotoxicity and hypoglycemia, muscle and cardiac fatigue, and cellular energy
  • Nutrients and biochemistryB vitamins, D3/K2, iron, omega-3/6 and cholesterol
  • Immune system and toxinsChronic inflammation, autoimmune conditions and toxic load
  • EndocrineThyroid gland
  • Mental and emotionalSocial, emotional and spiritual dimensions

Beyond functional medicine, the library includes a general clinical base (structured on Mexico's NOM-004 clinical record standard), gynecology, pediatrics, geriatrics and mental health.

Pre-visit patient participation

Use the visit to assess, not to take notes.

Before the appointment, patients log in to their portal and complete the sections you have enabled for them. When they arrive, that information is already part of their clinical record.

  1. BeforeThe patient logs in to the portal
  2. BeforeCompletes the clinical history
  3. DuringThe clinician reviews and validates
  4. DuringSupplements and assesses

Part of the information gathering happens beforehand. Time with the clinician is spent understanding.

Questionnaires and previous assessments

Not everything you need to know fits in a clinical history.

The clinical history provides the context. Questionnaires collect and assess specific information with the instruments your practice uses. Oolara keeps both around the same patient.

Clinical history · context

The patient's history and structured information, organized by section and area.

Questionnaires · assessment

Instruments that are scored automatically and return a result with its interpretation.

One patient record. More information should not mean more scattered information.

From many answers to organized information.

Oolara scores each questionnaire according to the logic of its own instrument and presents the result, with the interpretation that instrument defines, for your review. Results can be viewed on screen and downloaded as a PDF.

Oolara organizes, scores and presents. The clinician interprets.

Follow-up and history

Don't look only at how the patient is today.
Review how things have changed.

Many care processes accompany the patient for weeks or months and reassess along the way. Oolara keeps the result of each administration in the patient's record: when you open a questionnaire you see how many times the patient has answered it before and when, and you can review earlier assessments during follow-up.

  1. 1 · First assessmentThe questionnaire, scored
  2. 2 · InterventionPrescriptions, lab orders and supplementation
  3. 3 · Follow-upNew appointments and ongoing care
  4. 4 · New assessmentThe same instrument, again
  5. 5 · HistoryThe new result, kept with the earlier ones

A single visit shows a moment. The history helps you review the process.

The library

A deep clinical history doesn't have to be complicated.

You don't start from a blank page. Oolara includes a library of clinical sections and questions already written and organized by area: you choose and configure the ones your practice needs.

60 sections
18 families
7 clinical areas

The depth is already structured. You decide what you need to use.

Configuration by clinician

Your clinical history should match the way you practice.

Not every clinician needs to ask the same questions, and even within one specialty people work differently. The depth functional medicine requires led us to build a history that can adapt, and that same flexibility serves other ways of practicing.

  • Enable what you need, hide what you don't use
  • A different history for each clinician in the same practice
  • Validity period and age range per section
  • Clinician-only groups that the patient never sees
  • A record from another practice, only if it is shared with you

Built for depth. Flexible enough to adapt.

Clinical record documents

Everything about the patient, in one place.

Each patient's record holds the clinical history, the questionnaires with the result of each administration, and the documents generated at every visit: visit summary, prescription, lab order and supplementation, all in PDF with your professional license number.

And what the system does not generate: study results, previous documents and other files that you upload, or that patients upload from their own portal. Relevant patient information doesn't always arrive inside a form: it arrives as the PDF the laboratory sent, a note from another physician or a study from three years ago.

Already have your own clinical history form?

Your clinical experience defines what you need to ask. On every plan you compose your history from the existing sections. From the Growth plan, the Forms builder lets you create your own clinical history sections and questionnaires: the questions, how they are answered and how they are scored. You publish them and they are available right away, with no quote and no waiting. If you prefer guidance, our implementation team can review it with you.

Other specialties

Not a functional medicine practice? You can use it too.

You don't need a functional history, questionnaires or longitudinal follow-up to benefit from the platform: configure the history with only the sections your specialty needs. Gynecology · Internal medicine · Endocrinology · Pediatrics · Nutrition · General and family medicine, among others.

Oolara is built for the depth of functional medicine, but it doesn't require you to practice it.

Then, practice operations

The clinical history is only the beginning.

Patient information doesn't live apart from the rest of the practice: from the same record, everything that happens around that care stays connected.

  1. ClinicalHistory and questionnaires
  2. ClinicalAppointment and visit
  3. ClinicalPrescriptions, lab orders and supplementation
  4. OperationsProducts and inventory
  5. OperationsPayments and Mexican invoicing (CFDI)

More comprehensive care also needs more comprehensive practice management.
